I. Informacje ogólne.

1. Operatorem serwisu www.orthos.pl i jest ORTHOS SZPITAL WIELOSPECJALISTYCZNY Sp. z o.o. Komorowice, ul. Wrocławska 2A 52-229 Wrocław
2. Serwis realizuje funkcje pozyskiwania informacji o użytkownikach i ich zachowaniu w następujący sposób:
   1. Poprzez dobrowolnie wprowadzone w formularzach informacje.
   2. Poprzez zapisywanie w urządzeniach końcowych pliki cookie (tzw. „ciasteczka”).
   3. Poprzez gromadzenie logów serwera www przez operatora hostingowego.

II. Dane osobowe:

Dane osobowe są przetwarzane na podstawie rozporządzenia Parlamentu Europejskiego i Rady (UE) 2016/679 z dnia 27 kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych osobowych i w sprawie swobodnego przepływu takich danych (Dz. Urz. UE z 4.5.2016, L 119/1) oraz ustawy z dnia 10 maja 2018r. o ochronie danych osobowych.
3. Na potrzeby formularzy kontaktowych mogą być zbierane następujące dane osobowe: imię, nazwisko, adres e-mail, numer telefonu.
4. Celem przetwarzania danych osobowych pozyskanych w formularzach kontaktowych jest nawiązanie kontaktu oraz przekazywanie informacji handlowych, marketingowych i promocyjnych.
5. Udzielenie zgody na przekazanie danych osobowych w formularzu kontaktowym jest dobrowolne. Brak udzielenia zgody nie może powodować żadnych negatywnych konsekwencji dla użytkownika, a jednie może utrudnić lub uniemożliwić kontakt z użytkownikiem ze strony administratora.
6. Osobie, której dane dotyczą przysługuje prawo do:

1. W celu realizacji praw, o których mowa w punkcie II 6 powyżej osoba, której prawa dotyczą może kontaktować się z Administratorem pisemnie lub elektronicznie korzystając z ogólnodostępnych danych kontaktowych Administratora.

1. Osobie, której dane dotyczą przysługuje prawo do wniesienia skargi do Prezesa Urzędu Ochrony Danych Osobowych.
2. Cofnięcie zgody na przetwarzanie danych osobowych nie wpływa na zgodność z prawem przetwarzania, którego dokonano na podstawie zgody przed jej cofnięciem.
3. Dane osobowe przetwarzane na podstawie udzielonej zgody będą przetwarzane do czasu jej odwołania.

III. Informacje w formularzach.

1. Serwis zbiera informacje podane dobrowolnie przez użytkownika.
2. Serwis może zapisać ponadto informacje o parametrach połączenia (oznaczenie czasu, adres IP)
3. Dane w formularzu nie są udostępniane podmiotom trzecim inaczej, niż za zgodą użytkownika.
4. Dane podane w formularzu są przetwarzane w celu wynikającym z funkcji konkretnego formularza, np. w celu dokonania procesu obsługi zgłoszenia serwisowego lub kontaktu handlowego.
5. Dane podane w formularzach mogą być przekazane podmiotom technicznie realizującym niektóre usługi – w szczególności dotyczy to przekazywania informacji o posiadaczu rejestrowanej domeny do podmiotów będących operatorami domen internetowych (przede wszystkim Naukowa i Akademicka Sieć Komputerowa j.b.r – NASK), serwisów obsługujących płatności lub też innych podmiotów, z którymi Operator Serwisu w tym zakresie współpracuje.

Pliki cookies (tzw. „ciasteczka”) stanowią dane informatyczne, w szczególności pliki tekstowe, które przechowywane są w urządzeniu końcowym Użytkownika Serwisu i przeznaczone są do korzystania ze stron internetowych Serwisu. V. Logi serwera.

1. Informacje o niektórych zachowaniach użytkowników podlegają logowaniu w warstwie serwerowej. Dane te są wykorzystywane wyłącznie w celu administrowania serwisem oraz w celu zapewnienia jak najbardziej sprawnej obsługi świadczonych usług hostingowych.

VI. Udostępnienie danych.

1. Dane podlegają udostępnieniu podmiotom zewnętrznym wyłącznie w granicach prawnie dozwolonych.
2. Dane umożliwiające identyfikację osoby fizycznej są udostępniane wyłączenie za zgodą tej osoby.
3. Operator może mieć obowiązek udzielania informacji zebranych przez Serwis upoważnionym organom na podstawie zgodnych z prawem żądań w zakresie wynikającym z żądania.

   2. the Act of November 6, 2008 on Patients’ Rights and Patient Ombudsman (i.e. OJ of 2017, item 1318, as amended), secondary legislation thereto, the Act of December 5, 1996 on the Profession of Physician and Dentist (i.e. OJ of 2018, item 617), the Act of July 15, 2011 on the Profession of Nurse and Midwife (i.e. OJ of 2018, item 123), and other legal acts in the provision of health care services;

The recipients of your personal data are the entities referred to in Article 26(3)(1-12) of the Act on Patients’ Rights and the Patient Ombudsman and the processing entities (entities entrusted with the processing of personal data within the scope of: inspection, repair and servicing of medical instruments and equipment, servicing and maintenance of IT systems, medical consultations, commissioning of medical examinations and services).
6. Your personal patient data is kept for a period of 20 years, counting from the end of the calendar year in which the last entry was made, except as provided in Article 29 (1) (1-4) of the Act on Patients’ Rights and Patient Ombudsman.
7. In connection with the processing of your personal data, you have the right to access the content of your data, to rectify, erase, restrict its processing and portability, as well as the right to object, and other rights in this regard under applicable law.
